    Introduction

Disulfiram (Antabuse) is a drug used for the treatment of alcohol dependence, for alcoholism prevention, and as treatment of severe alcohol abuse (). The drug is the most commonly prescribed form in the US and Europe (). Antabuse is the brand name of the drug Disulfiram and is manufactured by the pharmaceutical company Merck & Co. It is available in 250 mg and 500 mg tablets.

The drug is sold under the brand name Antabuse. However, there are some differences between the brand name drug and the generic drug (disulfiram). The active substance is disulfiram, which belongs to the family of drugs called Antidepressants (). The drug is also sold as a generic form of Antabuse (Antabuse-generic). The generic drug is a drug containing the same active ingredient as the brand name drug, and thus is equivalent to the brand name drug.

The mechanism of action of disulfiram in alcohol dependence has not been fully elucidated. The pharmacokinetic and therapeutic results of disulfiram-alcohol and disulfiram-alcohol-alcohol in humans show a steady increase of plasma concentration following a single oral dose, while the concentration after a short-term oral dosing with disulfiram is significantly decreased in the drinking-out period (, ). Therefore, the drug has a low potential for addiction, especially in individuals with a history of alcohol addiction.

However, disulfiram-alcohol-alcohol is not recommended for the treatment of severe alcohol abuse (). Disulfiram-alcohol-alcohol-alcohol is indicated as an effective treatment option for alcoholism when it is prescribed only after proper assessment of the individual's medical history and the current treatment with a single daily dose. The treatment of severe alcohol abuse should be started in a way that ensures a satisfactory and successful outcome of the treatment, and is usually started at the same time. The patient should be monitored for signs and symptoms of alcoholism, such as decreased interest in activities or loss of interest in activities, excessive drinking, or an inability to enjoy a particular type of food. The maximum dose should not exceed 500 mg of disulfiram in one dose.

The drug has to be discontinued when the patient is not able to maintain abstinence. In the present study, we investigated the effect of disulfiram-alcohol-alcohol on the pharmacokinetics of the drug and to assess the therapeutic effects of the drug in individuals with severe alcohol use disorder.

    Materials and methods

    Animals

A total of 10 healthy adult male subjects (age 18–55 years, age range 28–65 years, mean age 39.1 years) participated in the study. All subjects had normal physical and psychological assessment and were receiving alcohol with no other medication.

All subjects were admitted to the outpatient clinic for assessment of the severity of alcoholism and the medical history of alcohol use disorders before and during the treatment with disulfiram-alcohol-alcohol.

    Injections

Injections were given intravenously (IV) to the subjects at a dose of 250 mg (0.5 mL) by a sterile syringe. The dose was maintained for up to 24 h, and the effect was observed for up to 2 weeks after the injection.

After the injection, the subjects were advised to consume the drug by mouth without drinking alcohol and were instructed to drink only a single glass of alcohol a day. The dose was increased daily, with no change in the duration of the treatment. The dosage of the drug was gradually increased according to the body weight, until the maximum recommended dose of the drug was reached.

The dose of disulfiram-alcohol-alcohol was fixed at 250 mg in a glass of water. The dose was gradually increased with the body weight of the subjects, and the maximum recommended dose was reached within 12 h after the administration of the drug.

The study was approved by the institutional review board of the National Ethics Committee and all subjects provided written informed consent before enrolment.

During the treatment, a series of IV injections were given by a sterile syringe. The dose of disulfiram-alcohol-alcohol was increased by 250 mg every day. The maximum recommended dose was reached after the administration of the drug.
